Based on the memoirs of Jennifer Worth; the story follows twenty-two year old Jenny, who in 1957 leaves her comfortable home to become a midwife in London's East End. She is surprised to find that she will be living in a convent: Nonnatus House. Working alongside fellow nurses and the medically-trained nuns, Jenny has her eyes opened to the harsh living conditions of the slums, but she also discovers the warm hearts and the bravery of the mothers. Even after Jenny leaves Nonnatus, she continues to chronicle the lives of the midwives who have become her family.

In 1957 newly-qualified but naive midwife Jenny Lee comes to work in London's East end to work with an order of nursing nuns when she had been expecting a hospital as a base. Whilst pleased to have helped no nonsense Sister Evangelina deliver her first baby she is shocked by the poverty of her clients,terming the women 'heroines'. She is especially concerned for Conchita Warren,a Spanish former child bride,who speaks no English and is about to give birth for the twenty-fourth time. When Conchita falls down,inducing early labour, Jenny is thrown in at the deep end to deliver the baby on her own but wins praise from the doctors for her successful work.

Elderly,senile Sister Monica Joan,prone to wandering around the area shoeless,is arrested for shop lifting and appears in court. Jimmy proves to be the hero of the hour by driving to Chichester to collect the head of the order,Mother Jesu Emanuel,whose speech on behalf of the old lady leads to her acquittal. Jenny then realises that she has been a fool to think that she could have a relationship with the married Gerald who would never leave his wife for her.

After delivering a baby in a toilet Jenny is approached by a ragged old woman asking after the child's welfare. The nuns identify her as Mrs Jenkins,who lives in terrible poverty but refuses help. Eventually Jenny and Sister Philomena get through to her and help her improve her life. Jenny is perplexed that Mrs Jenkins mistakes her for somebody called Rosie and uncovers a tragic set of events in the old lady's life before closure is achieved. Rehearsing for the local nativity play its director Chummy seeks the help of intelligent school leaver Lynette Duncan,unaware that the girl is heavily pregnant,ultimately giving birth but leaving the baby at the convent door. Chummy unites mother and child but the play seems doomed when its costumes are destroyed when a water pipe bursts. Fortunately the locals rally round to ensure that the show goes ahead.

As Christmas and the wedding of Dr Turner to Shelagh, the former Sister Bernadette, approach an unexploded World War Two bomb is discovered in the area, requiring the residents to be evacuated overnight to the working men's club, where the nurses take charge. Among the evacuees are young marrieds the heavily pregnant Yvonne and her husband Alan for whom the bomb recalls terrifying memories of hand to hand combat in the Korean war. He is psychologically damaged and Trixie, her own father having suffered from shell shock, empathizes. Despite opposition from the nuns she and Jenny ensure that Alan is present at his child's birth to make him feel wanted and give him confidence. Further anxiety is caused by a polio outbreak with the doctor's son Timothy among the victims but there are no fatalities and the bomb is detonated. It does however destroy the convent and new premises are found as nuns and nurses finally attend the wedding.

Now dating surveyor Alec Jenny joins the other midwives in their superior new base, along with latest recruit Sister Winifred. Jenny is concerned for Merle and Billy, a young couple whose infant boys are permanently ill, causing them to doubt their parenting skills whilst added is stress is provided by the fact that Billy's brother died as a toddler. Surprisingly Sister Monica Joan accidentally provides Dr Turner with the solution to the children's complaints, thus saving them. Chummy meanwhile drums up support for the newly opened but under-used ante-natal clinic, taking advantage of a royal visit to put it in the spotlight. The day is a success and to add to it Chummy helps an older woman with a difficult birth, resulting in Sister Julienne asking her to return to work.

Jenny is promoted to acting sister at the clinic, irking the more experienced Trixie. Expectant mother of three Doris Aston also seems to prefer Trixie to Jenny though Doris has a bigger problem - the child that is due was not fathered by her bullying husband Cyril but by a black man at a one-night stand. Sister Julienne arranges for Doris to go away for the birth and have the baby adopted but the child arrives early and Cyril finds out. The baby is taken away for adoption but Jenny agrees to keep a letter for her from Doris explaining the situation for when she is older. Elsewhere Cynthia, inspired by the progressive Dr Latham, impresses cynical Sister Evangelina by putting into effect the doctor's relaxation technique for a difficult home birth.

However she is concerned that pregnant Jewish Leah Moss's married life is dominated by caring for her mother Tzirale, who has not left their flat in twelve years. Leah believes Tzirale is still traumatised by events in Europe during the war but the arrival of the baby helps the matriarch to overcome her fear, allowing the family to move, whilst Sister Winifred regains her confidence having delivered the baby on her own. Jenny is set to spend a weekend in Brighton with Alec but he is fatally injured in a fall. Shelagh, now resigned to the fact that she cannot have children, channels her energies into organizing a choir, who sing at the funeral.

The father, cerebral palsy sufferer Jacob, lives in the same residential home and they declare that they are in love and aim to marry but the baby is stillborn and Jacob is sent to another home. News of Evangelina's celebration attracts her brother Vincent, now an alcoholic tramp, and she steals food for him and sees him admitted to hospital, but her party goes ahead - despite her protests - and the community shows their appreciation of her work over the years.

After consoling Chummy Sister Monica Joan is reminded of her rift with her own mother decades earlier and becomes distressed. Chummy and Peter decide that they will nurse the terminally ill Lady Browne at their house, helped by the nuns and midwives. She becomes appreciative of their efforts, becoming less hardened and more loving to Chummy before she dies whilst Sister Monica Joan also benefits from her part in helping. The Turners have double good news as they are finally accepted as adoptive parents and Shelagh's choir wins the local competition. Jenny meets Philip Worth, cousin of a patient, whom she will eventually marry and leaves the area to work at a hospice, though she will keep in touch with her 'family' and will never forget them.

One couple who would have benefited by it are Gina and Leslie, who only married because she was pregnant with Leslie showing little interest in upcoming fatherhood. Bargee's wife Daisy Blacker, whose itinerant life-style has denied her children schooling, is reluctant to leave her barge and brood to have her latest but her commitment to her children and Leslie's to his wife are tested during a violent storm bringing Phyllis and Patsy out on call. Phyllis surprises Patsy with an admission while next morning the storm has passed and the calm brings with it a welcome face returning.

As Christmas 1962 approaches Sister Julienne gets a call from the Hope Clinic, a mission hospital in South Africa, in danger of closure due to under-staffing on the eve of a much needed polio vaccination programme. Along with a group of midwives, the Turners, Fred and Tom, she arrives at the clinic to assist its lone medic, Myra Fitzsimmons, and they are shocked by the lack of supplies and the rudimentary conditions, with Phyllis delivering a baby under a tree in the desert. They also encounter Apartheid and dysentery, caused by the lack of clean water. Worst of all Dr Myra is convinced she has cancer. Before they return to England Trixie has performed a Caesarean, Sister Julienne has persuaded a gruff white land owner to run a fresh water pipe-line through his property and Dr Turner has good news for Myra, all allowing the clinic to continue. At the same time Barbara and Tom have good news of their own
